Domiciliary Care Co-Ordinator

  • Job title: Domiciliary Care Co-Ordinator
  • Responsible to: Deputy Head Manager
  • Job purpose: To recruit suitable carers to work with service users and to co-ordinate packages of care.
  • Company information: The Company is registered with the Care Standards Inspectorate for Wales as a large Domiciliary Care Provider (over 200 hours per week).
  • General

    1. To work within the policies and procedures of New Directions paying special attention to the policies which promote a safe working environment for all staff and visitors.
    2. To contribute to the overall success of the Agency by attending team meetings, etc.
  • Candidates

    1. Following the Agency’s policies and procedures for the recruitment of Domiciliary Care staff.
    2. Plan, meet and write care plans and needs assessments for new clients.
    3. To meet with new clients and communicate changes, introductions of new candidates.
    4. To review and spot check any package to ensure compliancy to the care standards.
    5. To assist in writing and implementing Risk assessments in relation to clients needs.
    6. Update all records on personnel file and Adapt.
    7. Ensuring all candidates files are fully compliant in line with Care Standards legislation before being passed to Manager for signing off.
    8. Ensuring continual support to candidates and supervisions, training, etc are kept up-to-date.
    9. To assist in payroll and payments of timesheets for all Dom care packages
  • Co-ordinating Care Packages

    1. Liaising with Social Workers to ensure smooth running of Domiciliary Care Packages.
    2. Working with other consultants to ensure consistent cover on all packages.
    3. Matching suitable candidate’s skills with the requirements of the clients.
    4. Ensuring care plans and all relevant paperwork is in place for new packages before commencement in line with Agency and CSIW or local statutory requirements.
    5. Ensuring on going support to clients, quality assurance checks and care plan/risk assessments are all up to date.
    6. Undertaking complex Risk Assessments.
    7. Undertake out of hours duties on a rota basis
  • Business

    1. To contribute to the success of the Agency by engaging new business.
    2. To promote the Agency in a positive way within the Domiciliary Care Sector and wider business community.
    3. Participate in the out of hours service on a rota basis.
